After arriving at our offices, our check-in staff will be greeting you and asking you to fill a small questionnaire. In case you’re a returning patient, we will ask for an update of your records in case you changed your phone number, address, or insurance coverage. This allows us to stay current and give you pertinent attention and information at all times. Additionally, we will ask you for your insurance ID card to keep copies along with your personal records.
We accept most health plans and insurances. It is essential to bring your insurance ID card in every visit to our center. If you have any change in your insurance coverage, be sure to notify our front desk staff to maintain your medical records updated.
